LM-activators have long been established as a convenient and physiological tool for correcting malocclusion in children. However, in recent years, there has been growing interest in the use of these devices in adult patients.

How effective are they? In what cases can LM-activators be beneficial for adults? We explore this in this article.

What is an LM-activator

How does an LM-activator work, and for whom is it typically suitable?

An LM-activator is a soft, dual-action silicone mouthpiece designed to correct the position of the teeth and jaws. It combines the functions of:

  • an orthodontic trainer (jaw growth control);
  • a retaining device (maintaining results after braces);
  • a myotherapeutic device (muscle retraining).

The most pronounced effect of LM-activators is seen in children, especially when disorders are detected early. They can be used to correct:

  • mouth breathing;
  • abnormal tongue position;
  • forward/backwards and width jaw misalignment.

They are especially effective between the ages of 6 and 12, when the jaw is actively growing and the permanent bite is forming. That is why correcting the bite in children using LM-activators is considered the gold standard in modern preventive orthodontics.

Features of bite correction in adult patients

In adult patients, the jaws are already formed. This significantly complicates any orthodontic intervention. Also in adults:

  • tooth wear is more common;
  • there are secondary bite deformities;
  • there is an increased risk of periodontal disease;
  • mobility of the TMJ may be limited.

Correcting the bite in adult patients requires a comprehensive approach. A single mouthguard is usually not enough — the participation of an orthodontist, therapist, and sometimes a maxillofacial surgeon is required.

When is the LM-activator effective for adult patients?

Despite its limited capabilities, the LM-activator can be beneficial in some instances for adults.

Retention after braces treatment

One of the main tasks of the LM-activator in adults is to maintain the result after brace removal. It helps stabilise the position of the teeth, reduces relapses and maintains correct occlusion.

Thanks to its soft material and physiological shape, the tray is well tolerated, does not cause trauma to the mucous membrane and also helps normalise muscle tone.

For TMJ dysfunction and increased tooth wear

LM activators are often prescribed to patients with complaints of:

  • clicking in the joints;
  • discomfort when opening the mouth;
  • headaches, tension;
  • nighttime bruxism (teeth grinding);
  • pathological tooth wear.

In these cases, the LM-activator works as a myofunctional device, relieving overload on the TMJ, redistributing the load and ‘retraining’ the muscles of the face and neck. It does not replace complete treatment, but can be part of a comprehensive therapy.

Limitations and disadvantages of the LM-activator in adults

Unfortunately, the LM-activator cannot replace full orthodontic correction in adults with severe malocclusion. Here are the main limitations:

  • Does not work as a primary treatment for severe malocclusion.
  • It does not move teeth like braces do.
  • It cannot correct asymmetries, deep bites or crossbites.
  • It may cause discomfort in people with a strong gag reflex or narrow dental arches.

Additionally, the patient must be highly motivated and disciplined, as effectiveness depends directly on the amount of time the appliance is worn (approximately 10-12 hours per day).

Alternatives to the LM-activator for adult patients

  • Orthopaedic treatment — in case of tooth loss or pathological wear.

  • Do not rely on aligners as a magic pill. In adult patients, results are only possible with strict adherence to the wearing regimen, correct diagnosis and regular monitoring.

  • Do not use the activator if you have active gum inflammation, severe periodontitis or temporomandibular joint disorders without the permission of a specialist.
