Our jaw is a dynamic, living structure that is constantly remodeling itself. To remain dense and robust, the bone requires constant pressure. When we chew, the load is transmitted via the tooth root to the alveolar ridge. For the body, this serves as a functional signal to direct nutrients and minerals to this area to maintain bone density.

As soon as a tooth is extracted, this natural cycle is interrupted, and the following processes are triggered:

  1. Impaired microcirculation: The capillaries in the area of the missing tooth constrict, and blood flow to the periosteum decreases. Without nourishment, bone cells begin to die off.
  2. Halt in regeneration: Bone-forming cells (osteoblasts) cease to function, whilst bone-resorbing cells (osteoclasts) continue their activity, literally dissolving the unused tissue.
  3. Volume loss: In the first year after extraction, the bone can lose up to 25% of its width, and over time, it begins to rapidly decrease in height as well, turning into a thin ‘ridge’.

It is important to understand: conventional removable dentures do not solve the problem. These devices are fixed to the mucous membrane, exerting external pressure on the gums without stimulating the deeper layers of the jaw. Such pressure does not stimulate the bone from within; on the contrary, it compresses the blood vessels, accelerating atrophy. Only high-quality implantation, which replaces the tooth root, can truly halt bone loss.

How can bone atrophy change your appearance?

Atrophy is not just a problem confined to the oral cavity. The jawbone acts as a framework supporting all the soft tissues of the face. When this bony support is lost, the facial muscles and skin sag, distorting the face’s natural contours.

Facial ‘ageing’ and the appearance of premature wrinkles

In aesthetic medicine, there is a term known as ‘dental ageing’. A reduction in the volume of jawbone tissue is immediately reflected in one’s appearance, triggering a number of characteristic changes:

The main challenges for implant placement in cases of bone deficiency

For conventional implant placement, a certain amount of bone is required so that the implant is securely surrounded by tissue on all sides. If there is insufficient ‘foundation’, there is simply nowhere to anchor the implant.

Main obstacles:

  1. Proximity of the maxillary sinuses: in the upper jaw, where there is a bone deficiency, there may be only a couple of millimeters of tissue remaining between the oral cavity and the sinus. To safely place an implant, a sinus lift in Kyiv (or another city) is often required.
  2. The mandibular canal: a major nerve runs through the lower jaw. Severe atrophy makes it impossible to place a standard implant without the risk of damaging this nerve.
  3. Narrow alveolar ridge: the bone may become so thin that the implant is wider than the bone itself.

Modern methods of bone grafting and sinus lift
