Aesthetic Smile Restoration with Zirconia Crowns

In brief: the patient wanted a harmonious smile but declined orthodontic treatment. With crowded teeth, a deep overbite, failed old composite restorations and significant discoloration, we performed an aesthetic rehabilitation with full-zirconia crowns using Digital Smile Design. A full-zirconia crown is milled from a single block of zirconium dioxide, with no ceramic veneer and no metal core. The absence of a veneer rules out chipping, and the material’s high strength (flexural strength around 900–1200 MPa) withstands chewing loads. Modern multilayer (“translucent”) zirconia has translucency close to enamel, so the crowns look natural and show no grey line at the gum.

Stage 3. Digital Smile Design (DSD) and preparation

We performed a Digital Smile Design of the future smile, which let us preview the shape, proportions and position of the future teeth and approve the result with the patient. We then carried out the preparatory stages: minimal necessary preparation and temporary restorations.

The whole work was done in 10 days. The prognosis is high: systematic reviews report a 5-year survival of monolithic zirconia crowns of around 97.6–98% (higher than metal-ceramic, ~92%). Hygiene and bite control matter for longevity; with a deep overbite, a night guard may be recommended.
