All-on-4 implantation concept

In 1998, Dr. Paulo Malo successfully treated the first patient with the All-on-4 treatment concept. Since then, hundreds of thousands of patients have been treated using this technique.
The All-on-4 treatment concept is a cost-effective, graftless treatment solution that provides patients with a fixed full-arch prosthesis on the day of surgery, supported by four implants In the edentulous jaw, we will place two parallel implants in the mandible anterior region and two implants tilted up to 45º in the lateral surface. On the same visit, we will fix a temporary orthopedic construction to the implants, consisting of 10-12 teeth. Thus, bone augmentation (sinus lift) can be avoided by tilting the lateral implants and using the residual bone.

STAGES OF IMPLANTATION AND PROSTHETICS ACCORDING TO THE ALL-ON-4 PROTOCOL
1. The first step is to make a computed tomography scan for the planning of implantation and prosthetics.
2. Next, we will take impressions and manufacture temporary crowns for you, fixed to the implants.
3. On the third step, we will install 4 implants and immediately screw temporary crowns to the implants.
4. After 4 months, we will produce permanent crowns with 4 implants.

    However, even such an effective method of prosthetics is not free from some shortcomings.

    1. It cannot be used with severe atrophy of the jawbone, since in this case, even an inclined position of the implants will not give the necessary strength of the prosthesis attachment.
    2. If one of the four implants fails, it is impossible to use the old orthopedic design and, accordingly, it is necessary to replace the implant and make new prostheses.

    In case of severe and moderate atrophy of bone tissue, we recommend using alternative methods of prosthetics.

    All-on-3 protocol.

    This is another protocol introduced by Nobel – also called “Trefoil.” It is used only on the lower jaw and is based on only 3 dental implants installed in the anterior region parallel to each other. It is an alternative to All-on-4 since it is used if it is impossible to install implants distally at an angle.

    All-on-6 protocol.

    This method is used in the case of moderate bone atrophy. The principle of treatment is based on the All-on-4 concept but has some differences from it. Since the number of implants, in this case, is increased by two, the prosthesis is fixed more reliably. The All-on-6 protocol is also carried out in case of the periodontal tissues’ severe inflammation against the background of periodontitis.

    1. Will 4 implants withstand a 12-tooth structure? Such designs have been manufactured since 1998. Statistics show that such constructions last as long as conventional structures supported by 6 or 8 implants.
    2. Is it possible to use old dentures as temporary crowns? Yes, it is possible. In this case, the old prostheses must be adapted after implantation and then fixed to the installed implants.
    3. Are implants placed without gum incisions? Sometimes we make a surgical template to insert implants without incisions and immediately fix temporary crowns to them. This method is the least traumatic, although more expensive, and requires several days of preparation.
